Hemiplegia

4 Personal history

male patient, 50 years old, student, from Cairo, single, No special habits of medical importance, Rt. handed.

4 c/o

Heaviness of right upper and lower limbs of 6 days duration.

4 HPI

-The condition started 6 days ago by tingling and numbness of the Rt. Upper and lower limbs including the face ,.

- 2 hours later, the patient experienced weakness of gradual onset and progressive course, This weakness affected the right upper and lower limbs affecting distal more than proximal muscles, to the extent that patient can walk with support (hemiparesis)

-The condition was followed by difficulty of speech but the patient said the he can understand the words of the other (motor dysphasia)

-the condition was followed by deviation of angle of the mouth toward the left side with drip ling of saliva from the right side and accumulation of food behind the right cheek with intact eye closure

-no history suggestive of convulsions

-no history suggestive of disturbed conscious level

-no history suggestive of fever

- no history suggestive of manifestation of increase intracranial tension

-no history suggestive of other cranial nerve affection

-no history suggestive of transient ischemic attacks

-no history suggestive of similar attacks

-no family history of similar attacks

-patient is diabetic, hypertensive, ischemic heart disease

4 Past history

- History of D.M, HPN, drugs or operations.

4 Family history

- No similar condition in family.

- No consanguinity.

- No common diseases in family.

4 General exam

- Temperature: 37^o c.

- Bl. Pressure: 120/80.

- Pulse: regular, 70 beat/minute, average volume, no special character, vessel wall not felt, equal in both sides with intact peripheral pulsation.

- Mentality: The patient is fully conscious, well oriented for time, place and person. Average mood and memory. The patient is co-operative with average intelligence.

4 Examination of Speech: motor dysphasia

4 Examination of Cranial Nerves: Right U.M.N facial and hypoglossal

4 Examination of Motor System :

- No skeletal abnormalities, no trophic changes, no involuntary movement, no muscle washing.

4 Examination of Tone__

- There is normal tone in left upper and lower limbs.

- In right Upper and lower limbs there is hypertonia in the from of spasticity affecting antigravity muscle

-

4 Percussion__

No fasciculation or myotonia.

4 Examination of Muscle Power

- There is normal muscle power in lt upper and lower limbs.

- In right upper and lower limbs: there is weakness affecting distal more than proximal muscles, abductor more than adductor muscles, in the right Upper limb extensors are weaker while in the right . Lower limb flexors are more weaker (Indicating weakness of D tract distribution in the right Upper and lower limbs).

4 Coordination

- Cannot be examined on right . side because of weakness

- On light . side there is normal coordination proved by finger to nose, finger to finger, finger to doctor's finger in both eye opening and eye closure.

4 Reflexes

- Deep reflexes

� There is normal reflex in left upper, lower limbs.

� In right . upper & Lower limbs: hyperreflexia with +ve pathological reflexes.

- Superficial reflexes

� +ve Babniski on right . side and normal planter response on the left side.

� Abdominal reflexes: lost on the right side.

4 Sensory:

- Superficial sensation: Right sided hemi-hypothesia (complete i.e. face is included ).

- Deep sensation: intact

- Cortical sensation: can't be examined .

4 Back: no deformities, no pigmentation, no scars, no swellings.

4 Gait: circumduction gait(cause of this gait ) .

4 other system examination.

4 Investigation

CT, MRI.

4 Treatment

- ischemic or hemorrhagic

- Thrombolytics

- Anticoagulant

- Antiplatelets

- Stimulants

- Dehydrating measures

- Antihypertensive

- Oral hypoglycemic drugs

- Antihyperlipedemic

- Muscle relaxant

- Antidepressant

- Tonics

4 Diagnosis :

4 1) clinical diagnosis :-

Right sided Hemiplegia ( spastic ) ,motor dysphasia ,right U.M.N facial ,right U.M.N hypoglossal ,diabetic ,hypertensive

2) anatomical dignosis :-

Capsular hemiplegia

3)pathological diagnosis :-

Ischemic stroke

4) Aetiological diagnosis :-

Cerebrovascular stroke ,most propably ischemic stroke

v 4 differential diagnosis :-

v Prognosis :-

v Cause of death in stroke :-

v Difference between ischemic and haemorrhagic stroke

v Difference between embolic and thrombotic stroke

v Stroke and infective endocarditis

v Venous stroke

v Stroke in young age

وحاولت اكتب ورا الدكتور الشرح بالعربى والترجمة للمصطلحات
وهوا ساعات كان بيقرا من ملزمته فعملت للأجزاء دى
copy and paste
وخصوصا للجداول

فى ملف word

حوالى 23 ورقة

بس الملف ده للى عايز يفهم مش للحفظ لانى منقلتش كل الملزمة
الاصلية للدكتور يعنى بنقل منها الاجزاء بس اللى كان بيقراها بنفسه من ملزمته
واللى عايز يحفظ بعد كده ممكن يحفظ من ملزمة الدكتور الاصلية
بس الشرح بتاع الدكتور جميل جدا وهوا بيقول ايه المهم للامتحان وهوا بيشرح
